Therefore, the first step is always to meet with the new patient to determine problems, needs, and goals that particular person brings to us. The right kind of treatment can then be planned accordingly.
Services offered by Communicare can be divided into three types: outpatient treatment, residential treatment, and consultation. Any individual patient may receive one or more of these general types.
Outpatient Services: Outpatient care is similar to health services provided by a family practitioner in that a variety of problems are treated in the office setting. Outpatient services include: diagnosis and assessment, psychiatric evaluation, psychological evaluation, treatment planning, individual psychotherapy, group psychotherapy, family therapy, medication maintenance, school-based treatment of children and adolescents, specialized individual and group therapy for dually diagnosed patients, and forensic evaluation.
Case Management: Case management services are non-treatment adjunctive services designed to assist seriously mentally ill adults and seriously emotionally disturbed children function appropriately and attain greater benefit from clinical services. Case managers make home visits to teach family members how to assist in the patient's treatment, to observe family interactions, to assess patient functioning in non-office settings, and to assess patient compliance with treatment recommendations. Case managers assist patients in identifying and securing needed health and social services. Case managers serve as a "bridge" between the patient and the treatment professional.
Emergency Services: Emergency services are available 24 hours a day. Professional staff are always available to talk with patients via the telephone. Face-to-face services can be provided at any time if needed.
