MAP is short for “Making A Plan”. The MAP team is made up of individuals from local community agencies and parent representatives that work with and represent children and youth. The first priority of MAP team is to review cases concerning children and youth, (ages 0 to 21), who have serious emotional/behavioral disorder or serious mental illness and are at risk for an out-of-home placement due to the lack of access or availability of needed services and supports in the home and community. MAP Team children do not have to be clients of Communicare.
MAP Team Coordinator receives the referral form from a variety of sources in the community. The referral is presented and discussed at the monthly MAP Team meeting so that the team can decide if the child would benefit from MAP Team services. If it is determined that the child could benefit from MAP Team services, the MAP Team develops a plan to meet the identified needs of the child and family. The children/youth and their families are encouraged to attend the MAP Team meetings. At this time community resources are identified and recommendations are made to divert out home placement.
